Sunny Pine Home Care is seeking reliable and committed Disability Support Workers to deliver high-quality support to NDIS participants living in Supported Independent Living (SIL) homes across Melbourne’s Northwest, Northeast and Western suburbs.
This role is suited to individuals who are able to maintain consistent availability in line with rostered care requirements, ensuring participants receive stable and reliable support.
About the Role
As a Disability Support Worker, you will support NDIS participants to live as independently as possible while maintaining dignity, choice and control in their home environment.
You will assist with daily living activities, promote community participation, and support individuals to work towards their personal goals in a structured, team-based support setting.
Key Responsibilities
Provide personalised support with daily activities such as meal preparation, medication assistance, personal care, and household tasks
Encourage independence, social inclusion, and skill development
Support participants with complex needs, including behaviours of concern
Follow individual support plans and assist participants to achieve their goals
Maintain accurate documentation and report any changes in participant needs
Build respectful relationships with participants, families and allied professionals
Deliver support in line with NDIS Practice Standards and organisational policies
Availability & Commitment (Important)
Sunny Pine Home Care operates 24 hours a day, 7 days a week. Successful applicants must be able to commit to the availability provided during recruitment, which will form the basis of roster allocation.
To support continuity of care for participants:
Staff are expected to maintain their agreed availability after commencement
Rosters may include day, evening, overnight, weekend and public holiday shifts
Ongoing employment and shift allocation are dependent on maintaining reasonable availability for rostered shifts
We understand circumstances can change and will consider reasonable requests for adjustments where appropriate, however frequent or ongoing changes to availability may impact roster allocation and role suitability.
About You
You are someone who:
Demonstrates empathy, patience and professionalism
Is reliable and committed to providing consistent support to participants
Values teamwork and clear communication
Is comfortable supporting people with varying support needs, including mental health or dual diagnosis
· Understands the importance of continuity of care in a residential support environment
Experience in Supported Independent Living (SIL) or residential disability support is highly regarded.
Qualifications & Requirements
Certificate III or IV in Disability, Individual Support or similar (or working towards)
Current NDIS Worker Screening Check
Working With Children Check
Valid First Aid & CPR (HLTAID011)
Current Australian Driver’s Licence
Evidence of COVID-19 vaccination (if required by organisational policy or health guidance)
Experience with manual handling and personal care is desirable

What We’re Looking For:
Important: As continuity of care is critical in Supported Independent Living environments, applicants will be asked to confirm their ongoing availability during recruitment. Applicants should only apply if they can reasonably maintain the availability they nominate for roster planning purposes.
Sunny Pine Home Care is seeking Disability Support Workers who demonstrate the following qualities and commitments:
• A strong understanding of person-centred practice and active support principles, supporting participants to build independence, choice and control.
• A collaborative team approach, with the ability to work effectively alongside colleagues, families, and allied health professionals.
• A commitment to delivering support using a strengths-based approach, focusing on participants’ abilities, goals and capacity-building.
• Availability Requirements (Important):
o Minimum availability of three full weekdays (e.g. 8:00am–5:00pm)
o Availability to work shifts across 7:00am–11:00pm
o Capacity to complete up to two sleepover shifts per week (approximately 6:00pm–8:00am)
o Ability to maintain the availability provided during recruitment, as rosters are developed based on staff availability to ensure continuity of care for participants.
• A proactive and engaged approach to supporting participants, including actively interacting, communicating and building meaningful relationships.
• A genuine interest in supporting participants to achieve their goals, build life skills and participate in community activities.
• The ability to research, plan and support participation in community activities, events, courses or volunteer opportunities aligned with participants’ interests.
• The flexibility to adapt support approaches to suit the individual needs, preferences and personalities of participants.
• A commitment to following organisational policies, procedures and NDIS Practice Standards to ensure safe and high-quality service delivery within the framework of the National Disability Insurance Scheme.
• Strong time management and reliability, including punctual attendance at rostered shifts and timely completion of required documentation and incident reports.
• A professional and respectful approach to the workplace, contributing positively to team culture.
• The ability to plan and communicate leave requests in advance where possible, recognising the importance of consistent support and continuity of care.
