This is not your average disability support job. It’s a rare opportunity to combine hands-on support with leadership and coordination — helping one man live a thriving, meaningful life in his own home and community.

We’re seeking a Practice Lead – Disability & Community Support to join a self-managed “Service for One” support team.

About the Role

This long term commitment casual role (approx. 24 hours per week) combines:

  • Direct support – 1 weekly shift (8 hrs) providing person-centered support.

  • Leadership & coordination – 16 hrs per week working alongside the Director of JDC Support guiding staff and ensuring life runs smoothly.

  • Administration

You will support a 33-year-old man who lives independently in his own home, enjoys music, walking, cooking, gardening, and running a microbusiness. He is autistic, has an intellectual disability, and epilepsy — so knowledge in these areas is essential.

Key Responsibilities

  • Lead with purpose – mentor and support a small team, building their skills in communication, relationships, community connections,and well being.

  • Coordinate supports – liaise with therapists, community organizations, and family to ensure plans and strategies are implemented.

  • Hands-on connection – build a trusted relationship through direct support, community activities, and everyday routines. This enables you to understand our operation, get to know our client, get to know your staff.

  • Grow opportunities – help expand his microbusiness, deepen community connections, and create new opportunities.

  • Partner with leadership – work directly with the Director on operational tasks, team meetings, recruitment, and administration.

  • Multi Disciplinary Team - Work alongside our multi disciplinary team so you understand our NDIS goals and how the Team implement these and data collect

Who We’re Looking For

Essential:.

  • Strong relationship-building and communication skills.

  • Leadership or mentoring experience.

  • Flexible, problem-solving approach.

  • Strong Administration and co ordination abilities - very well organized

  • Administration and Compliance

    • Regulatory Compliance: Monitoring and interpreting legislation, regulations, and agreements to ensure compliance, particularly regarding the NDIS and work health and safety (WHS)

    • Ability to assist us to register for NDIS Provider (under way)

    • Must have full understanding of Policies and Procedures and implementation of such with the Team

Desirable:

  • Positive Behaviour Support knowledge.

  • Community engagement experience.

  • Understanding of NDIS processes and documentation.

Practical requirements:

  • Driver’s licence, NDIS Worker Screening, First Aid & CPR.

  • Epilepsy management experience (or willingness to learn).

  • Understanding of mental health and complex individuals
