ABOUT YOUR ORGANISATION

Westpeak Support Services is a NDIS-focused disability support provider operating in Victoria. We position ourselves around helping people with disabilities achieve greater independence, community participation, and positive outcomes from their NDIS plans through personalised, person-centered support.

ABOUT THE ROLE

As a Support Worker, you will provide person-centred care and support to clients in accordance with their individual care plans, helping them achieve greater independence, wellbeing, and community participation. Key responsibilities include:

  • Delivering high-quality support tailored to each client's needs, goals, and preferences.
  • Assisting with personal care tasks, including hygiene, grooming, dressing, and mobility support.
  • Providing domestic assistance such as cleaning, laundry, and household maintenance.
  • Offering respite care to support clients and their families/carers.
  • Encouraging and facilitating social support and community engagement activities.
  • Assisting clients with transport to appointments, shopping, social outings, and community events.
  • Supporting meal planning, preparation, and promoting healthy living practices.
  • Facilitating group participation programs and recreational activities.
  • Assisting clients to develop and maintain independent living and life skills.
  • Building positive, respectful, and professional relationships with clients, families, and stakeholders.
  • Promoting client choice, dignity, inclusion, and independence at all times.
  • Maintaining accurate records and reporting in line with organisational policies and procedures.

We have SIL properties in the South, Southeast, West and Inner city of Melbourne

ABOUT YOU

Do you have the following:

  • Qualifications in Disability, or a related field, with substantial experience providing direct care to people who have a Disability and require personal care?
  • First aid and CPR certificates?
  • Experience with Huntington's Disease, ABI, Autism, Epilepsy, FND, mental health?
  • Communicate effectively with a compassionate approach with a diverse range of participants?
  • Availability and flexibility to undertake various shifts (day, afternoon and overnight)?
