We are seeking compassionate and dedicated Community Support Workers to support NDIS participants across Melbourne's Western and Northern suburbs. Through person-centred support, you'll help participants achieve their goals, build confidence, develop independence, and engage meaningfully in their communities. About the Role As a Community Support Worker, you will provide person-centred support that enables participants to live independently, achieve their goals, and engage with their community. Your duties may include: Supporting participants with daily living activitiesAssisting with community access and social outings Transporting participants to appointments, education and activities Encouraging independence and life skills developmentProviding personal care support where requiredPromoting participant choice, dignity, and inclusionMaintaining accurate documentation and progress notes What We're Looking For About You: Caring, reliable, and passionate about helping othersA strong communicator with a positive attitudeAble to work independently and as part of a teamFlexible and committed to delivering high-quality support Essential Requirements NDIS Worker Screening Check (or willingness to obtain)NDIS Worker Orientation Module Certificate Current Working with Children Check -Employee Current First Aid & CPR CertificateCurrent Australian Driver LicenceReliable vehicle with comprehensive insuranceCurrent National Police CheckSmartphone and basic computer skills Desirable Certificate III or IV in Individual Support, Disability, Community Services, Mental Health, or similarPrevious experience in disability support, aged care, mental health, or community services Why Join Careforce Group?
