Our participant cohort includes individuals with:
ABI, stroke recovery, bariatric care needs, MS, MND, palliative care requirements, and complex disabilities. Participants access a combination of Equal Living supports alongside their own SIL providers for daily care needs.

The Role

We are seeking an experienced Team Leader – Disability Support to lead palliative care delivery across our four SIL/SDA facilities.

This role combines hands-on clinical care with team leadership and mentoring. You will work autonomously within a supportive environment, guiding and upskilling our disability support workforce while ensuring exceptional end-of-life care.

Key Responsibilities

  • Assess and manage palliative care needs for NDIS participants with complex neurological conditions
  • Develop individualized end-of-life care plans integrated with existing support frameworks
  • Provide clinical guidance, mentoring, and training to Disability Support Workers
  • Liaise with external palliative care providers, GPs, and allied health professionals
  • Ensure compliance with NDIS Practice Standards and medication management protocols
  • Travel between facilities across metro Melbourne as required

About YouEssential

  • Postgraduate qualification in palliative care OR demonstrated advanced experience
  • Certificate III or IV in Disability
  • Experience supporting participants with neurological conditions, ABI, or complex disabilities
  • Demonstrated ability to mentor and upskill non-clinical staff
  • Current Driver’s Licence and willingness to travel
  • NDIS Worker Screening Check (Required)

Highly Regarded

  • Experience in NDIS service delivery
  • Medication management certification
  • Experience in SIL/SDA environments
  • Clinical leadership background
