The Opportunity

We’re looking for a warm, organised, and people-focused Care Partner to join our Greensborough team. In this role, you’ll be the primary point of contact for a portfolio of clients receiving Support at Home services — helping them live safely and independently in their own homes by coordinating person-centred care that reflects their goals, preferences, and values.

If you’re experienced in community care or looking to step into a care coordination role, we’ll support your development every step of the way.

What You’ll Do

•    Welcome new clients through a thorough onboarding process — conducting assessments, gathering documentation, and ensuring they feel informed and supported from day one.

•    Develop person-centred care plans in genuine partnership with clients, their families, and representatives, ensuring plans reflect individual goals, risks, and cultural needs.

•    Monitor and review care plans on an ongoing basis, adjusting services as needs evolve and escalating clinical concerns to our nursing and allied health team.

•    Manage client budgets within the Support at Home framework, ensuring spending aligns with approved guidelines and maximises available hours and services.

•    Conduct regular client touchpoints, collecting feedback and building trusted relationships over time.

•    Coordinate care transitions between services — including hospital, respite, Transition Care Program, and ongoing Support at Home — ensuring continuity and clear communication.

•    Act as the first point of contact for clients, families, GPs, allied health professionals, and external providers, providing prompt and solutions-focused communication.

•    Maintain accurate and timely documentation in our care management system (Visual Care), supporting audit readiness and data-driven decision-making.

•    Contribute to quality improvement initiatives, reflective practice, and team discussions aimed at lifting the standard of care we deliver.

•    Stay informed about the Aged Care Act 2024, the Strengthened Quality Standards, and Support at Home program requirements.

What You Bring

•    Experience in community care, aged care, disability support, or a related human services field (preferred but not essential — we value attitude and potential).

•    A strong understanding of consumer-directed care principles and person-centred approaches.

•    Cultural competence and a genuine respect for diverse communities, identities, and life experiences.

•    Excellent organisational and time management skills with the ability to work autonomously.

•    Strong interpersonal skills and the ability to build trusting relationships with clients, families, and colleagues.

•    Comfort with technology — including care management software, digital tools, and standard office applications.

•    Knowledge of aged care regulations and the Support at Home program (advantageous).

•    A valid Australian Driver’s Licence.

•    National Police Check (or willingness to obtain).

•    Full work rights in Australia.
