About the Role

The Intake and Assessment Officer plays a key role in coordinating intake, conducting initial assessments, and supporting the implementation of NDIS and home care services.

This role also supports workforce coordination by maintaining staff records and assisting in matching the right staff to the right participants to ensure quality, compatibility, and continuity of care.

This is a fast-paced position requiring strong attention to detail, sound judgement, and the ability to manage competing priorities.

Key Responsibilities

  • Act as the first point of contact for NDIS and home care enquiries
  • Conduct initial needs assessments and identify appropriate support services
  • Coordinate referrals, intake documentation, and service agreements
  • Maintain participant records, data entry, and waitlists
  • Provide administrative support to the Program Manager / Director
  • Support efficient administration of NDIS and home care programs
  • Conduct or assist with home visits where required
  • Maintain and update staff files, ensuring compliance documentation is current
  • Assist in matching participants with appropriately skilled and compatible staff
  • Support workforce coordination to ensure the right client is matched with the right staff member
  • Participate in organisational and community events
  • Identify and implement intake and administration process improvements
  • Maintain accurate, confidential information management systems
  • Undertake additional duties as required

What You Will Bring

  • Knowledge of health, disability, and home care service systems
  • Understanding of participant needs assessment and workforce coordination principles
  • Experience navigating NDIS processes and service agreements
  • Experience in home care coordination and service delivery (desirable)
  • Strong Microsoft Office skills (Excel, Word, Outlook) and ability to learn new systems quickly
  • Excellent written and verbal communication skills
  • Strong organisational and time management skills
  • Ability to exercise sound judgement when matching staff skills and participant needs
  • Commitment to high-quality participant outcomes and continuous improvement

Selection CriteriaQualifications

  • Certificate IV in Disability, Community Services, Management, Social or Human Services

Experience

  • Minimum two (2) years’ experience in a similar role
  • Experience within an NDIS program, healthcare, or home care setting
  • Experience maintaining staff compliance records and supporting workforce coordination (desirable)

Pay: $30.00 – $40.00 per year
