A bit about the role…
The primary purpose of this position is to deliver case management to transitioning ADF Members and provide advice and guidance on achieving their goals for engagement in the community. To further ensure ADF Member is linked to an appropriate clinician for therapeutic supports, and assist them to transition effectively into civilian life including providing recommendations for employment and education and community engagement options.
This role will be responsible for providing one-to-one support to eligible ADF Members with complex medical conditions to transition from the ADF to civilian employment or meaningful engagement. The T4ME program is a vocationally focused service to deliver specialised employment support to members with complex needs, without encroaching on their clinical treatment plan. You will have a growth mindset and value supporting vulnerable people to achieve quality employment and social outcomes.
This role is permanent full time, based in Melbourne.
