What You'll be doing? (Essential Duties)
<ul>
 <li>Provide person-centred support to participants living in a Supported Independent Living (SIL) environment</li>
 <li>Assist with personal care tasks including showering, dressing, grooming, toileting, and daily living activities</li>
 <li>Support participants with complex health conditions including vision impairment, respiratory conditions, neurological support needs, and mobility limitations</li>
 <li>Provide safe mobility guidance and environmental support for a participant with total blindness and mild cerebral palsy</li>
 <li>Monitor participant wellbeing and promptly respond to any health or behavioural concerns in line with care plans</li>
 <li>Support medication routines and follow health management plans as instructed</li>
 <li>Facilitate community access, appointments, and social participation</li>
 <li>Maintain accurate documentation including progress notes, incident reporting, and shift handovers</li>
