We're seeking an experienced Team Leader - Support at Home to join a growing national organisation delivering person-centred community aged care services across a diverse multicultural client cohort.
This is an excellent opportunity for someone with strong case management experience who has previously led Case Managers and enjoys mentoring, coaching and building high-performing teams.

As Team Leader, you'll oversee day-to-day operations across the Support at Home program while leading and supporting a team of Case Managers/Care Partners and admin staff to deliver high-quality, person-centred services.
Your responsibilities will include:
* Leading, mentoring and supporting a team of Case Managers/Care Partners
* Providing supervision, coaching and regular performance feedback
* Supporting care planning, case reviews and complex client matters
* Monitoring KPIs, workflow and service quality outcomes
* Ensuring services meet compliance and quality standards
* Building strong relationships with clients, families and internal stakeholders
This is an office-based role in Roxburgh Park.
About youYou're an experienced community aged care professional who enjoys developing others, improving systems and leading from the front.
We're looking for someone with:
* Strong case management experience across HCP, CHSP or Support at Home
* Previous experience leading or supervising Case Managers in a similar role
* Proven ability to mentor, coach and support staff
* Strong organisation skills and ability to manage competing priorities
* Excellent communication and stakeholder engagement skills
* Bilingual skills highly regarded (but not essential)
* Salary: $95,000 + 12% super
* Full-time permanent role
