DISABILITY SUPPORT COORDINATOR:

Core Responsibilities:

  • Plan Implementation: Help participants understand and use their NDIS plan effectively, linking goals to supports.
  • Provider Connection: Identify, negotiate with, and connect participants to suitable mainstream and disability service providers.
  • Budget Management: Assist with managing NDIS funding and understanding costs.
  • Goal Tracking: Monitor progress towards goals and evaluate if current supports are working.
  • Relationship Building: Develop strong networks with participants, families, carers, and service providers.
  • Crisis Management: Respond to urgent needs and resolve conflicts.
  • NDIS Navigation: Maintain knowledge of NDIS policies, processes, and available community resources.

  • Tech Savvy: Proficiency with computers and Client Management Systems (CMS).
  • Independence: Ability to work autonomously.

Essential Requirements:

  • Qualifications: Diploma or Cert IV in Disability, Community Services, or 3 years experience in the field.
  • Experience: Case management, disability sector, or community health experience.
  • Checks: Current Police Check, Working With Children Check, and NDIS Worker Screening Check.
  • Licence: Current Driver's Licence and ability to travel.

Job Type: Part-time

Pay: $40.00 – $45.00 per hour
